Asthma

The asthma program helps members learn how to manage their asthma and improve their quality of life. Through ongoing partnership, collaboration, and phone calls, our experienced clinical care managers, including respiratory therapists, provide education about asthma and support for complying with each member’s doctor’s plan of care.  

Chronic Obstructive Pulmonary Disease (COPD)

Our COPD program helps members manage their disease. Our goal is to support members in practicing recommended self-care behaviors and following their physicians’ plan of care.

Diabetes

The diabetes program helps members learn how to manage their diabetes and reduce the risk of developing complications from their disease. The program consists of educational materials sent by email, telephonic coaching, education about glucose monitors and the importance of having regular office visits with their physician to have their A1C and bloodwork checked. We also help members take advantage of their diabetes benefits. 

Chronic Kidney Disease

The chronic kidney disease program focuses on supporting members with stages one, two, and three chronic kidney disease (CKD) learn to manage their condition and reduce the risk of developing complications. The program educates members about lifestyle modifications and evidence-based guidelines for the monitoring and control of CKD risk factors. The program emphasizes the importance of having a personal physician to guide kidney health management, enhance kidney function and improve the member’s quality of life.  

Heart Disease

This is a program for members with coronary artery or ischemic heart disease. The program educates members about lifestyle modifications and evidence-based guidelines for the monitoring and control of cardiac risk factors, such as hyperlipidemia and hypertension. 

Heart Failure

The heart failure program educates members with heart failure about appropriate self-care strategies to minimize worsening of their condition. 

High Cholesterol

The high cholesterol program is an educational program for members who want to learn more about managing their cholesterol. The program educates members about lifestyle modifications and evidence-based guidelines for the monitoring and control of cardiac risk factors, such as high cholesterol. 

Hypertension and High Blood Pressure

Our high blood pressure program is an educational program for members who want to learn more about managing their blood pressure. The program educates members about lifestyle modifications and evidence-based guidelines for the monitoring and control of cardiac risk factors, such as hypertension. 

Metabolic Health

There are a lot of things our members can do to manage their metabolic health. People who are overweight are at greater risk for developing diabetes. Members can take advantage of one of the many value-added discounts that come with their membership. These include local fitness centers and more. They may also receive information on how to manage their metabolic health risk factors. This includes helpful information on eating healthy, exercising and losing weight and managing  blood pressure and cholesterol. We have a staff of experienced health specialists, including nurses, registered dietitians and certified diabetes educators. These staff members can answer any questions members have about their condition and how they can lower their risk. 

Migraine

The migraine program is for adults who suffer from severe, recurrent headaches. We provide information about the importance of having a personal physician to guide headache management. 

My Diabetes Discount Program

The My Diabetes Discount Program is available to our small group chamber members and is a value-based benefit program in which qualifying members will be eligible for a $0 copay for their insulin therapy. To qualify for the reduced insulin copay, members must meet specific criteria. These requirements must be maintained on an annual basis to continuously receive discounted benefits. 

*Available for BlueCross Chamber groups with 50 or more members only.

Members many receive educational materials by email, coaching phone calls and case management services, when needed.
