Health Plan
The health plan portion of your benefits helps to pay the costs of medical services such as:
- Sick and well visits with your doctor
- Lab tests and diagnostic procedures
- Screenings and immunizations
- Inpatient or outpatient medical treatment
- Care in a hospital, urgent care center or other place of care
How health coverage may vary
How your health insurance pays for these services depends on the terms of your specific plan. And coverage may not be the same for all types of care.
For example, your plan may cover the cost of preventive services 100 percent, while you may need to meet a deductible and then continue to pay a percentage of the cost for other services.
Other requirements may also apply. Some plans will only pay if you seek care from in-network doctors. Or you may need to get a referral or prior authorization before you go for treatment.
